Milestones in lymphangioleiomyomatosis research.

چکیده
L ymphangioleiomyomatosis (LAM), a slowly progressive disease affecting mostly young and middle-aged females [1], is one of the few orphan lung diseases that has attracted major attention in recent years from both clinicians and researchers [2], with rapidly evolving progress. With an estimated prevalence of one in 400,000 adult females aged 20–69 yrs for the sporadic form [3] and ,30% of patients with tuberous sclerosis complex [4] (tuberous sclerosis complex itself has a prevalence of one in 5,800 live births [5]), LAM is indeed a rare disease according to the European definition (prevalence was less than one out of 2,000 persons). However, similar to what has been witnessed for idiopathic pulmonary hypertension some years ago, the organisation and care of LAM patients has improved considerably. Reference centres have been established in many countries for the care of patients suffering from rare diseases, either for rare pulmonary diseases as a whole including LAM, or specifically for LAM (LAM clinics). One initiative funded by the 7th Framework Programme of the European Commission is in the progression to establish European networks of centres of expertise (coordinator: T.O.F. Wagner, Frankfurt, Germany; LAM coordinator: S.J. Johnson, Nottingham, UK). Furthermore, basic and translational research has made tremendous progress, and some medical therapy may be envisioned in the near future. Hence the hope is forming that LAM may no longer be orphan in the coming years; the sooner the better.

Pseudoneutropenia in lymphangioleiomyomatosis (LAM) patients receiving sirolimus: evaluation in a 100 patient cohort
In lymphangioleiomyomatosis patients receiving sirolimus treatment, transient leukopenia in the morning may be due to circadian rhythm, with leukocyte counts recovering later in the day, indicating that a decrease in drug dose may not be warranted http://ow.ly/jPFz30iysgV.
متن کاملEffects of sirolimus on Lung function in patients with Lymphangioleiomyomatosis
Background: Lymphangioleiomyomatosis (LAM) is a progressive lungs disease that affects women at reproductive years. Sirolimus inhibits mammalian target of rapamycin (mTOR) and its administration in past studies was hopeful in treatment of patients with LAM. The aim of this study was to evaluate sirolimus therapy on lung function in LAM patients. Methods: We conducted a trial to evaluate the ef...

Pulmonary lymphangioleiomyomatosis and abdominal angiomyolipoma are related lesions for which there is no authentic animal model. Both of these proliferative lesions occur in sporadic patients, and at much higher frequency in patients with tuberous sclerosis, which is due to mutations in the TSC1 and TSC2 genes. Tsc1+/- and Tsc2+/- mice frequently develop liver hemangioma. We found that the Tsc...
متن کاملUnderstanding the priorities for women diagnosed with lymphangioleiomyomatosis: a patient perspective
Lymphangioleiomyomatosis (LAM) is a rare lung disease that almost exclusively affects women and develops in about one in 400 000 adult females. The European Lung Foundation worked closely with one of the patient organisations within its network, the European LAM Federation, to raise awareness of LAM at the 2014 European Respiratory Society International Congress in Munich, Germany. In addition,...
متن کاملPulmonary lymphangioleiomyomatosis presenting as spontaneous pneumothorax treated with sirolimus - A case report
Spontaneous pneumothorax is a very common medical emergency. Patients are often treated without treating the underlying cause. Lymphangioleiomyomatosis (LAM) is a rare cystic lung disease. Until recently, diagnosis of LAM was a challenge with nearly 100% mortality in 10 years, but better understanding of the disease through research and better radiological techniques and newer drugs such as sir...
